A intradural Dermoid Cyst in Cauda Equina -A Case Report-

Abstract
Dermoids are congenital cystic tumors arising from embryonic rests. Although usually congenital in origin, there is an evidence to suggest that some dermoids may be iatrogenically produced.
In the spinal canal most occur in the lumbosacral region.
We have experienced a case of the intradural dermoid cyst at lumbar region, which was removed successfully and confirmed histologically.
A 17 year-old student was admitted to our neurosurgical ward with severe low back pain and radicular pain along posterior-lateral arpect of the right lower extr¡þemity since 2 months prior to admission. There was a history of appendectomy under lumbar spinal anesthesia a few years ago. On his admission, neurological findings were tenderness at L4 level with paravertebral muscular spasm and positive Laseque¢¥ s sign in 30 degree in the right side. Myelogram showed a downward concave filling defect at middle portion of the 4th vertebral body. Total laminectomy per¡þformed through L3 to L5 level as usual manner and found a ovoid, 1. 5x 1.5 in size,
cystic mass intradurally without adhesion to root fibers. Total extirpation of the cystic tumor was successfully carried out. Microscopic finding disclosed a dermoid tumor. His post-operative course was uneventful without aseptic meningitis. There was no evidence of recurrence on follow up observation.
